Clinic Policy:

HOME VISITS
Doctors may perform home visits when appropriate, and if time allows. Generally restricted to those who are a regular patient of our doctor and are too frail or ill to get to our surgery.

TELEPHONE CONSULTS
Doctors may perform telephone consults to our patients whom we have seen face to face for the last 12 months. Generally restricted to those who have any of the following symptoms: fever, respiratory symptoms such as cough, fatique, sore throat, headache or difficulty breathing. Regular patient of our doctor and are too frail or ill to get to our surgery.

PRACTICE COMMUNICATION POLICY
Messages are triaged and sent to doctor on duty. If the matter is urgent the doctor will advise what action is required. Other messages are checked by the doctor routinely during the session and actioned accordingly.

PATIENT TEST RESULTS
Where blood tests or other investigations have been ordered, you will generally need to make a followup appointment to discuss the results. If the doctor is concerned about your results, our staff will contact you to make an appointment. Only the doctors will be able to give you the results.

PATIENT RECALL FOR PREVENTATIVE CARE


We have implemented a Patient Register for Preventative Activities. This is a Reminder/Recall System in which we contact the patient by either mail or phone for follow up for the preventative activity such as a skin check, cholesterol check, blood pressure check, Pap Smear, Mammogram or Prostate examination etc. We also offer the opportunity for patients to initiate these reminders/recalls to be sent to them. The purpose of this register is purely as a service to patients from this practice and confidentiality is ensured at all times. If you wish to OPT OUT of our recall system, please tell your doctor.

YOUR PRIVACY
ACACIA RIDGE MEDICAL CENTRE respect your privacy. Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and too ensure that this information is only available to authorised members if staff. Please ask a member of Reception if you would like a copy of Privacy Policy.
